PROVISION-CERTIFICATE OF PROTEST OF INSTRUMENT-B
On [date signed], at the request of [holder of instrument], of [address of holder], [city of holder], [state of holder], I, [Name of Notary], a duly commissioned and sworn notary public, residing at [address of Notary], [city of Notary], [state of Notary], did during the business hours of this day, present the original copy of the instrument that is attached to this certificate to several persons at several places in [city of presentment], and did make diligent inquiries for the [maker or as the case may be] to demand payment on the instrument. I was unable to find [maker or as the case may be], and I was creditably informed that [maker or as the case may be] did not reside at the address designated in the instrument and that [maker or as the case may be] had no office or place of business at the address designated in the instrument.
